Checkbox doesn't show up

In this form there's a checkbox list at the end. The checkboxes don't show up.
I bought a licence of laytheme and the ninja forms as suggested on your website.

they are still there. Have a look here.

I think that LayTheme overwrites some CSS settings from Ninja Forms. With CUSTOM CSS you can probably style them like you want.
