Bug: Adjusting product quantity in the cart causes additional (form-) fields to appear.

When you change the quantity of a product in the cart and click "Update Cart" additional fields appear:

On the first change of the product quantity, an additional voucher field appears. With every further change to the product quantity, the product fields / product list entries (including voucher fields) double.

yea seems like woocommerce changed the way its replacing the form here after clicking update cart.
